You will need to use a terminal which can set the X11 urgency hint when
a bell occurs in the terminal.
Using urxvt, you put the following in .Xdefaults:
 URxvt*urgentOnBell: true

Now you have to set mcabber to bell when a message arrives:
 set beep_on_message = 1
