You are correct, changing the primary output using xrandr also makes
this output the first Xinerama screen. So this is a solution to the
problem for two screen setups.

Unfortunately this does not seem to be a solution. At least not to me
since the screen order as returned by Xinerama somehow seems to depend
on when I plug the monitors in. And since I use a Laptop that I carry
around and plug monitors in and out the order of the screens is
sometimes correct/as expected and sometimes not. So swapping keybindings
in dwm would not solve the issue.
