Yes, it is offscreen for some reason. I'll have a look at this

There is no special rule, because apps which don't have a ~ rule
or set the WM_TRANSIENT_FOR hint are managed, except the
floating layer has been focused.

Can you check with xprop(1) if the app has the TRANSIENT_FOR
hint set?
